1. Do you practice under a different name than your Facebook profile? If so, please indicate. This is used to verify you are a Canadian optometrist/ophthalmologist.
2. What province(s) do you currently practice?
   
3. Please indicate your designation. This group is currently only for woman-identifying optometrists and ophthalmologists.
